It is important that you are familiar with your healthcare policy. Including what is and what is not a covered benefit on your plan. If a referral is required for your office visit or in-office procedure it is your responsibility to contact your PCP (Primary Care Physician) with in an adequate time frame to ensure you have your referral for the date of your service with us. In the event you do not have your referral and/or your PCP is unable to fax it to us on the date of your service we will be happy to reschedule your appointment for a later date.


Obstetrical Patients:

We offer non-insured patients a payment plan for all your obstetrical services. Upon, your second visit to our practice you will meet with our office manager to set up this plan.

All payments are due by your 7th month.

The payment plan covers:

  • All obstetrical office visits
  • Delivery with one of our physicians/midwives at the hospital
  • 24 hour medical advice
  • Post Partum care 4-6 weeks following your delivery


The payment plan does not include the following, which are billed to you and payment is due on the date of the service:

  1. Ultrasounds
  2. Non-stress tests
  3. Emergency room visits or hospital visits
  4. Lab tests
  5. Circumcision
  6. Surgery either elective or medically necessary
  7. Hospital fees


Obstetrical Patients who wish to Transfer Into the Practice

We are delighted to welcome new patients to the practice. In the event you are pregnant, over 12 weeks pregnant, and under the care of another physician we require that you have all of your current prenatal records including lab work and ultrasounds (if applicable) faxed to 954-755-8315.


Upon receipt of your records you will receive a phone call confirming that they are going for review with our physicians and upon approval we will make an appointment with the physician of your choice.

Please include the following when you fax your prenatal record to us:

  1. Cover sheet including your name, contact phone number and name of health insurance you are on
  2. Complete prenatal record, inclusive of lab-work and ultrasounds

You will have a response within 48 hours of receipt of the fax. Should you have any other questions related to your transfer please contact, Shannon at extension 220.
